  • Direct clinical responsibility for your treatment will reside with a named, highly experienced Virtue Consultant Specialist of your choice.  You will receive an initial evaluation call with your Consultant to define your personalised Care Plan. Your Consultant will then oversee the delivery of the Care Plan, as well as being a point of escalation in the event of deterioration or exacerbations occurring. 

  • Virtue source best-in-class Remote Monitoring devices from global life science companies, to ensure our patients and clinicians have the very latest, most advanced and easiest to use technology, as it becomes available and certified.  All devices guarantee clinically validated accuracy, as well as being portable, stylish and unobtrusive.   

     

    We provide new devices to our patients every 12 months, all of which come with a12-month Virtue warranty.  This provides our patients with the very latest technological advances, ensures compliance with clinical regulations, and provides total peace of mind. 

  • Our highly skilled Virtual Nursing Hub operates 7 days a week, 365 days per year between the hours of 8am to 8pm.  Over 80% of our Virtual Nurses are Advanced Nurse Practitioners, each having a MSc in Nursing.   

     

    Our Virtual Nursing Hub perform the following duties for patients receiving our Elective Remote Monitoring services: 

     

    • Remote monitoring of patient vital signs  

     

    • Collection of Patient Reported Outcome Measures (PROMS) and questionnaires  

     

    • Rapid escalation management, leveraging advice & guidance from specialist doctors, with oversight by the named Consultant Specialist

  • Our aim is to improve the management of your condition, to reduce the frequency and severity of deteriorations from the baseline target in your Care Plan.  However, should you experience deterioration or an acute exacerbation, our Virtual Nursing Hub will manage the escalation.  Our Virtual Nursing Hub will identify deviations via their day-to-day monitoring of your vital signs, or in your reported symptoms.  Should you feel unwell, you may also initiate an escalation yourself by directly contacting our Virtual Nursing Hub.  In the event of escalation, our Advanced Nurse Practitioners will speak with you, conduct further evaluation and where relevant will contact a Specialist Doctor or your Consultant Specialist for advice and guidance.  This may result in changes to your treatment and Care Plan, or recommendation to be admitted to our Virtual Ward or hospital.

  • In the weeks that follow a clinical escalation, changes in your condition, or changes to your medication, we will increase your level of monitoring.  This provides you with closer observation, enabling us to evaluate and optimise your treatment, quickly and thoroughly.  Ultimately, this enables us to deliver better outcomes for you, or recommend the best course of action to seek the right treatment in a timely fashion.

  • During times of optimising your Care Plan, you will receive scheduled check-in calls with an Advanced Nurse Practitioner from our Virtual Nursing Hub.  These are 15-20 minute calls, during which our nurses will check how you are doing, ask questions to gain further insight into the status of your condition and effectiveness of the treatment plan.  The nurse may provide some coaching and support to help with adherence to the treatment plan and may make recommendations to the Consultant of ways to further optimise the treatment or medication.

  • In short, yes.  However, it should be noted that patients will require the support of their regular care team and in some cases may be need to provide additional support.  

  • Yes.  Owing to the high prevalence of hospital admissions of patients who are receiving domiciliary care, or residents of nursing and care homes, our Remote Condition Management service is highly relevant.  We reduce clinical risk from the homecare providers by identifying deterioration faster, which can either help manage and prevent the onset of an medical crisis, or enable earlier admission to our Urgent Acute Virtual Ward or an Intensive Care facility.   Not only does this reduce the risk for the homecare provider, but it enables continuity of their services.   • In the event that a patient's condition deteriorates while being monitored receiving our Remote Managed Care service, our escalation pathways ensure that the responsible clinical team are notified and engaged promptly.  Depending on the severity of the condition, escalation may involve increasing the frequency of monitoring, initiating a remote consultation or if necessary, recommending that the Patient is stepped-up into our Acute Virtual Ward, or a facility with an Intensive Care Unit.
